Please insure all contractors, sub contractors, repairmen or service providers register with the office located at 9 Scenic Ct. # 6 Branson Mo 65616, Phone 417-334-7339,

Fax 417-336-5974 prior to performing any work in your unit or on campus. Also our insurance company requires a Certificate of Insurance provided by their insurance (standard ACORD 25-S form) a sent directly to our office via email or fax.

 

The Certificate must include:

1. Additional endorsement for general liability naming the owner (Fall Creek Condo Owners Association).

2. Commercial General Liability:

• General Aggregate $1,000,000

• Product/Complete Operations Aggregate $1,000,000

• Personal/Advertising injury $1,000,000

• Each occurrence Limit $1,000,000

3. Worker's Compensation and Employer's Liability:

• Part one-Workers' Compensation: Statutory Limit

• Part two- Employer's Liability: Annual Limits:

Bodily Injury by accident, each Accident $500,000

Bodily Injury by disease, each Employee $500,000

Bodily Injury by disease, Policy Limit: $500,000
